TASTE THE DIFFERENCE

 

ASSUMPTION OF RISK:

Prepping, handling,  and cooking in a professional and non-professional kitchen carries with it certain intrinsic risks that cannot be eliminated regardless of the care taken to avoid problems. The specific risks vary, and can include but are not limited to cooking food thoroughly; separating and not cross-contaminating foods; chilling or refrigerating food appropriately; cleaning foodstuffs, instruments, and hands used in and for food preparation; fire or gas hazards.

 

WAIVER:

I, do hereby release, waive, discharge, and promise not to sue Personal Chef Aneva Walker, her affiliates, direct or indirect, from liability from any and all claims resulting in personal injury, accidents or illnesses (including death), due to cooking in respective establishments, as in homes, apartments, condos, etc. I understand that any foods requested to be served raw or undercooked carry risks of its own, and can cause illnesses or death. Any illnesses, accidents, or death due to unknown or expressed allergies are not at the fault of Chef Aneva Walker and therefore will not be pursued in or out of court for claims thereof.

I also understand that Chef Aneva Walker is not responsible for any expense or liability incurred as a result of my participation in hiring a Personal Chef or attending any events utilizing a Personal Chef, including medical expenses due to any sickness or injury incurred as a result. Persons with weaker immune systems, such as elderly or pregnant women, carry a greater risk for foodborne illnesses. I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.
